Description:
- Monitor residents’ status including vital signs, significant lab values, nutrition, and activity.
- Completes nursing assessments according to facility policies and procedures and standards of nursing care.
- Record all significant observations in the nurses' notes.
- Receives all new orders and follows up appropriately.
- Inform the dietary department of any new admission or dietary change of a current resident via the communication slip.
- Completes incident report when incidents occur on their shift; notifies physician and family members: and, start an incident investigation.
- Administers medications per physician orders, documents on the resident MAR and completes any other needed monitoring such as required blood pressures with medicine administration.
- Communicates with the interdisciplinary care plan team to insure that residents' needs are appropriately planned for in the care plan and the plan is resident directed.
- Completes skilled admission and discharge sheets including contacting the appropriate pharmacy.
- Maintains a safe environment for residents, visitors, and staff and is knowledgeable of OSHA, state and other regulatory requirements.
- Reduces the risk of healthcare-acquired infections through compliance with current CDC hand hygiene guidelines and the use of Standard Precautions.
- Ensures nursing procedures are followed by nursing service personnel, and it reflects the day to day nursing care procedures used by the facility.
- Directs the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the facility, including the making work assignments to nursing assistants.

Description:
- Document data related to patients' care including assessment results, intervention, medications, patient responses or treatment changes.
- Monitor, record and report symptoms and changes in patients' conditions including but not limited to vital signs.
- Perform emergency medical procedures, such as basic cardiac life support (BLS), advanced cardiac life support (ACLS), and other condition stabilizing interventions.
- Manage patients' pain relief and sedation by providing pharmacologic and non-pharmacologic interventions, monitoring patients' responses, and changing care plans accordingly as directed by the physician.
- Administer blood and blood product transfusions or intravenous infusions, monitoring patients for adverse reactions.
- Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
- Order tests to identify and assess patients' condition as indicated by the physician.
- Discuss illnesses and treatments with patients and family members.
- Monitor all aspects of patient care including diet and physical activity.
- Direct and supervise less-skilled nursing or health care personnel or supervise a particular unit.
- Assess the needs of patients' family members or caregivers and provide input or provide support for these individuals.
- Start IVs, administer injections and administer medications as indicated by the physician.
- Read current literature, talk with colleagues, and participate in professional organizations or conferences to keep abreast of developments in acute care.
- Supports and abides by the mission, values, and policies of CHCS in all activities.
- On-going compliance with CHCS educational requirements.
- All other duties as required.
